The Independence Assessment
Twenty questions, about three minutes. There are no right answers and nothing here is a test.
Answer honestly rather than optimistically. A score that flatters you is no use to anyone, least of all you.
Preparedness
I've thought through what staying independent at home might look like for me over the next 10 to 20 years.
I have a simple plan for what I would do if something unexpected happened and I needed help.
Important information (contacts, key documents, medication list location, etc.) is organized so someone could help if needed.
I've identified a few possible future challenges (mobility, stairs, driving, isolation, etc.) and begun thinking about solutions.
Home Environment
My home has good lighting in key areas like entrances, hallways, stairs, and bathrooms.
I've reduced common fall risks in my home (loose rugs, cluttered walkways, slippery surfaces).
My bathroom setup is reasonably safe (non-slip surfaces, stable supports, or plans to add them).
I've thought about how my home would work if I temporarily had limited mobility.
Strength & Mobility
I do some form of strength activity at least twice per week.
I include balance or mobility exercises in my routine.
I can comfortably perform everyday movements like getting up from a chair, climbing stairs, or carrying groceries.
I have a realistic routine that helps me stay active consistently.
Community & Support
If I needed help within 24 hours, I have at least one or two people I could call.
I regularly connect with friends, neighbors, or community groups.
I'm building (or already have) a small "village" of people or services I trust.
I've thought about how I would stay socially connected if circumstances changed.
Technology
I use at least one simple technology tool that makes life easier or safer.
I have a reliable way to reach help quickly if needed.
I'm comfortable using basic technology to stay connected with others (or I have someone who helps me with it).
I'm open to using simple technology tools that support independence as I age.
